I would appreciate if you can fill out a service waiver (for the specific service you are having) no more than 24 hours prior to your appointment. If you have had the service done previously you do not need to fill out the form again unless any information has changed.

If you are coming for a yearly touchup on your microblading or tattooed brows you will need to fill out a new waiver.
