I would appreciate if you can fill out a service waiver (for the specific service you are having) no more than 24 hours prior to your appointment. If you have had the service done previously you do not need to fill out the form again unless any information has changed.

If you are coming for a colour boost (yearly touchup) on your microblading/nano or ombre (powder) brows you will need to fill out a new waiver.
